- I understand this tool is designed solely to facilitate awareness, communication and understanding of my COVID-19 risk profile and that of others who share their profiles with me, to support transparent risk-informed decision-making as part of planning a safe visit with each other.
- I understand this questionnaire does not provide a health or medical advice, and it was not developed by physicians or health professionals. In no way does this tool supersede the guidance of Federal, state and local public health authorities; my medical doctors or my own judgement.
- I understand that it is prudent and courteous to calibrate my risk protocols to accommodate the highest-risk person in the group I am planning to visit with.
- I understand that I alone am responsible for the veracity/truthfulness of my responses and my actual behaviors and associated risk exposures to COVID-19 or any other communicable, transmissible disease. The usefulness and effectiveness of the questionnaire depends entirely on my truthfulness and that of the people I am planning to visit.
- I understand this tool is intended only for situations where parties already have trusted relationships.
- I understand this is a "point-in-time" questionnaire and that it is my responsibility to alert the other people I plan to visit with if anything at all changes regarding my possible exposure risks between the time I complete this survey and my planned visit.
- I understand that no data from the questionnaire is stored on any server, other than the basic contact information I entered to receive access to the questionnaire.
- I agree to indemnify and hold harmless the organization who provided me the COVID-19 SafeVisit web application, and AHA Communication, LLC, their agents, and/or employees from and against every claim or liability, suits, threats, demands, settlements, negotiation costs and expenses, other costs, including attorney fees, court costs, and expenses arising out of, or in any way related to, the use, misuse or unauthorized modification, reproduction, release, performance, display, or disclosure of data received in use of this tool by me, or any person to whom I have released or disclosed the data that may now or at any time in the future exist under any federal or state law.
